In 2018, physicians Simon Talbot and Wendy Dean named what was actually happening to clinicians: not burnout, but moral injury — the wound of being repeatedly forced to act against your own deepest values by a system you didn't design and can't control alone. Burnout suggests you're just tired. Moral injury names the truth: something in you has been wounded by betrayal, not depleted by hours.
If you're a physician and a leader, you have a second compression point most conversations never name. I have both lived and coached experience around this double crush, a concept borrowed from neurology, used here as a metaphor to illustrate physician leader moral injury.
In medicine, a double crush injury occurs when a nerve is compressed at two different points, causing symptoms far greater than either compression alone. Physician leaders experience something similar. The first compression is the healthcare system. The second compression is leadership itself, where you're expected to absorb organizational pressure while transmitting it to the people you lead. Together, those two forces create a deeper form of moral injury that burnout alone cannot explain.
As a leader, you don't just absorb the system's demands — you become the one who has to pass them on. You're the one who has to enforce the policy you don't believe in, deliver the productivity targets that hurt your team, defend decisions made above you to people who trusted you below you.
That's the second point of compression. That's why rest doesn't fix it, and why generic leadership training doesn't either. You're not under-resourced or under-trained. You're under-decompressed.
You can't wait until you're healed to lead well, and you shouldn't have to pretend you're whole while you do it.
